Covid-19 Proof of Vaccination

Thank you for joining Calvary and the Calvary community at our services and events.

Following the guidance of public health officials in San Francisco, we will continue to wear masks in worship and keep the distancing in the sanctuary. The change is that we will be requiring proof of vaccination for everyone 12 years and older at every event that involves food and for admission to children’s ministry gatherings.

You do not need to show proof of vaccination to attend worship.

Children under the age of 12 remain welcome to attend all Calvary events, and we hope these changes will keep them safer until vaccinations are approved for them.

You can use this secure, private form below to provide proof of your vaccination status. Once you’ve uploaded the information, a “Vaccine pass” will be emailed to you. You can then print it to show it or keep it on your phone and show us a digital version of it when you enter Calvin Hall or Sunday Studio.

You will be receiving an email from Charlene Thai within two business days after you upload your vaccine card. This email will include your vaccine pass. If you have not received an email within two business days, please email Charlene at charlenethai@calpres.org.
